Legal Form BL2 (blue) for employment problems such as contract terms and employment protection; dismissal, redundancy, racial discrimination, sexual discrimination and sexual harassment; equal pay; maternity rights or any other employment problem. Please complete in block capitals.

IMPORTANT WARNING
This form goes to Usdaw to request legal assistance. It does not lodge your
claim at an Employment Tribunal. To have your case heard at a Tribunal you must make a separate
application on form ET1. You can do this online at www.employmenttribunals.gov.uk, or obtain the ET1
form from your local Jobcentre Plus. You are responsible for making sure that your ET1 claim reaches
the Tribunal within the time limits. These are very strict and normally three months less a day from the
date of dismissal or cause of complaint. You should always follow the company grievance and appeal
procedures.
