Incident report form

It is important that we are aware of incidents that happen in Usdaw workplaces and this Incident Report Form has been produced to enable members to report any such instances. We need to gather this evidence to persuade Government, police and local authorities that there is a real problem of violence and abuse against shopworkers.
This report will be dealt with in the strictest confidence and none of the information you have given Usdaw will be passed to a third party without your express permission.
