Expression of Wish

Also often called a Nomination form, this is the form you must complete in order to nominate the person(s) you want to receive your lump sum death-in-service benefits.

The pension scheme trustees do not have to follow the instructions you give them but in the majority of cases they will do.

If your circumstances change (e.g. if you get divorced or remarried or have children) you must complete a new form so the trustees know what your wishes are.
