from our resources area.
Supplies of the forms
can be obtained from the Stationery Department at
Central Office and they should be easily available in every
workplace.
Do members need to
use the right form?
It is better to use the
correct form because the forms are designed to cover the essential
information. But a claim will not be rejected if it is not on the
correct form or if it is not on any form at all.

The forms to use are:
Form BL3 - pink - for legal
problems not connected to work, probate and conveyancing.
Form BL4 - free Wills form -
for free Will writing for member and partner.
Form BL6 - Grey - for
work-related prosecution cases. Prosecutions can also be dealt with
by telephone 0161 249 2477.

Forms
We have tried to make all
our forms user friendly. However, if any member needs help in
completing the Application Forms, they should contact their local
Rep or Branch Official for assistance.
If that is not possible, or
assistance is needed urgently, then members should telephone their
local Usdaw office or telephone the Usdaw Legal Department on 0161
224 2804.
How to get
assistance in employment cases
The Usdaw aim is to resolve
employment matters under the procedures in the workplace.
Employment cases will
usually be dealt with by Usdaw reps in the workplace supported by
the Area Organisers. The Usdaw Legal Department will support and
advise reps and officials.
If a grievance or a
disciplinary matter cannot be resolved under the workplace
procedures then the member may wish to make a claim to the
Tribunal.
Tribunal cases are usually
dealt with by the Area Organiser and the Usdaw Legal Department.
The Legal Department may refer some cases to our Panel of
Solicitors but that is unusual. The Head of Legal Services decides
whether and by whom the member will be represented.
If a member needs legal
assistance for an employment matter they must contact their
local
office for a Member Application Pack. In employment cases
members should always contact
their Area Organiser.
In employment cases it is
important that members follow the disciplinary and grievance
procedures in operation by their own employer.
Members and reps must also
be aware of the tribunal time limits and the statutory rules about
complying with disciplinary and grievance procedures.
